The health and well-being of every student is a main priority for our school.  Please find the health forms for the 2023-2024 school year below. Yeshivat He'Atid requires annual medical forms for every child. These health forms provide permission for your child to participate in gym and all school activities. All forms must include an up to date immunization record and date of the exam within the last year. Please do not submit camp forms.

Medical forms can now only be submitted by uploading the forms to the Parent Locker website. Therefore, we cannot accept hard copies or emailed forms. Please complete the forms via this link: https://yh.parentlocker.com/medical-form/ or via the Medical Forms tab on the left side of your parent locker account. Every child must have current medical forms and be up to date on their immunizations in order to attend school. We do not accept religious exemptions for immunizations.

Please upload your child’s most recent medical forms even if they have already been submitted via email during the past school year. All forms must be uploaded before your child can attend school. If your child has an appointment in August, you can wait until you have the most updated version of the forms and upload right after the appointment, as long as the forms are uploaded before school begins.

If your child is on a sports team and their forms expire before the sports season begins, updated forms will be required before they are able to play. We recommend that you take forms with you to be filled out at every well visit.

Please note:
Toddler through Pre-K: annual flu shots are required and documentation must be submitted by December 31st
Kindergarten: booster shots are required for DTaP, Polio, and MMR
6th grade: Tdap booster and Meningococcal vaccines are required


Required Forms:

Please choose one of the following options:

1- Health Record
To be used for Toddlers through Grade 5 (and Grades 6-8 if not playing on a sports team). This form includes permission for administration of over the counter medications. If your doctor's office will only fill out their own forms, please be aware that their forms may not include this component. You can bring the Over the Counter Medication Form below to be filled out if that is the case. This form is to be completed by the child’s pediatrician and signed by a parent. Annual flu shots are required for all Toddler, Nursery, and Pre-K students and must be received before December 31st.

OR BOTH OF THE FORMS BELOW:

2 - Preparticipation Physical Evaluation
Grades 6 through 8- If there is ANY chance that your child may participate on a sports team, this form must be filled out by you and your doctor in its entirety. Otherwise, please complete Health Record.

3 - Over-the-Counter Medication Form
This form needs to be signed by both a parent AND needs a doctor’s signature. No medication will be given without this form completed in full.


Elective Forms:

Allergy Action Plan
If your child has any allergies that may require Benadryl, Epinephrine, or other allergy medication at school, you and your doctor must complete this form.  It is critical that we have this information so that medication is not delayed.

Asthma Treatment Plan
If your child has chronic or even occasional asthma, please complete the form WITH your doctor.  There must be BOTH a parent and a doctor’s signature.

Medication Administration Form
We encourage you to schedule all medication to be taken at home.  However, if your child may require any medication during school hours, whether over the counter or prescription, please complete this form with your doctor. All medication must be sent to school in original pharmacy bottle.
